2. Processing of Personal Data
A visitor to the website or an applicant for information via form, email or other means, as a data subject, acknowledges that their personal data in the scope specified in the electronic form and data relating to the applicant for information or services will be processed by the Provider as the controller for the purpose of:
- Contacting the data subject
- Sending back a response or information
- Repeated contact and sending information about services
3. Legal Basis for Processing
The legal basis for processing personal data is, according to Article 6(1)(b) of Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, hereinafter referred to as "GDPR"), the fulfillment of the Provider's obligation to contact or send back a response or other information.
Important: Providing the above personal data is a requested requirement, and the legal consequence of not providing them is the impossibility of contacting or sending information.
4. Data Retention Period
Personal data will be retained for the period necessary for communication or sending supplementary or repeated information.
No automated decision-making or profiling takes place during the processing of personal data, and data is not sent to third parties.
5. Rights of the Data Subject
The data subject has the following rights:
- Right of access - the right to request from the Provider confirmation as to whether their personal data is being processed
- Right to rectification - the right to rectification of incorrect or incomplete personal data
- Right to erasure - the right to erasure of personal data if one of the reasons specified in the GDPR is met, in particular if personal data is no longer necessary for the purpose for which it was collected
- Right to restriction - the right to restriction of processing of personal data if one of the reasons specified in the GDPR is met
- Right to data portability - the right to obtain personal data concerning them and which they have provided to the controller in a structured, commonly used and machine-readable format and have the right to transmit this data to another controller
- Right to lodge a complaint - the right to lodge a complaint with the relevant supervisory authority

The data subject may contact the controller in connection with the processing of personal data and exercise their rights:

7. Processors and Data Recipients
Personal data will be processed on behalf of the Provider by its employees who perform various tasks related to the organization of services.
The Provider has entrusted some processing operations to third parties (processors who process personal data on behalf of the controller, to the extent and under the conditions agreed with the controller). Processors in the processing of personal data are persons who ensure the technical operation of the website through which services are provided.
8. Processing of Patient Data
About the processing of patient personal data by the Provider, which occurs in connection with the provision of healthcare, or within the provision of other services or sale of goods, patients are informed:
- During the provision of healthcare
- By notice when loading the website
- When providing other services or selling goods
9. Consent to Processing
By filling out and submitting the form, the website visitor and sender/applicant for information confirms consent to these terms.
I consent to the processing of personal data for the purpose of handling the request and subsequent communication in accordance with the privacy policy. The request can be canceled/disagreed by canceling the form submission.
